The New World of Cell Therapy and Orthobiologics: A Needle and Not a Knife, Part 1

 My goal is to separate fact from fiction by delivering evidence-based care specifically related to orthobiologics, and contribute to biologics knowledge and application in arthritis and orthopedic conditions. At present, too many patients are navigating the field independently without reliable sources of information resulting all too frequently in substandard care provided by non-orthopedic physicians; thus, experiencing unsatisfactory outcomes. I still am an orthopedic surgeon having evolved into regenerative medicine using stem cells, proprietary platelet rich plasma, growth factors, exosomes and acellular amniotic fluid with a needle and not a knife.

It was a little over three years ago when we first met. At that time, I could not walk through a Walmart without experiencing significant pain in my left knee. Well, since that time my knee has continued to improve. Fast forward to today, we have just returned from our hiking trip to the southwest national parks (Grand Canyon, Bryce, & Zion). The patient who sent me this beautiful expression of appreciation had presented to my office seeking to postpone, perhaps avoid a Total Knee Replacement. He had already been treated for several years by a local physician with physical therapy, non-steroidal anti-inflammatory medication, and injections of cortisone and hyaluronic acid to no avail.  After assessment and review of images, I provided him informed consent regarding the several options available at the time; he elected to undergo a stem cell procedure using Adult Mesenchymal Stem Cells harvested from his bone marrow. Several days prior to the bone aspiration, a Platelet Rich Plasma injection using the patient’s own platelets was completed to initiate healing. Five days later, the bone marrow aspiration was completed in a surgi-center to assure sterility and the concentrated bone marrow containing Stem cells, Platelets, Precursor Cells and Growth Factors were injected into the patient’s left knee under image guidance. About a week later, a concentration of a proprietary Platelet Rich Plasma filtrate was injected into the left knee under image guidance as well. The result is best described the patient himself in the above message quoted above.

What I have described took place three plus years ago. Since then, the biologic options have increased; in part owing to my clinical research and that is what I want to explore with you in this webinar.

Options for Patients with Delayed Joint Arthroplasty Due to COVID-19

Because of the COVID-19 crisis, many surgeries have been cancelled or delayed for patients who qualified for, or had scheduled a total joint replacement or other orthopedic surgical procedure. Post COVID-19, operating rooms will be occupied by other urgent procedures (cardiac, cancer) and many elective procedures will be backlogged. In other words, you may have to wait many months for an elective orthopedic surgical procedure. I am able to provide patients with another option; a procedure that is supported by strong scientific evidence, minimal risks and complications, can be easily performed in the office, and that is FDA cleared for use in orthopedics. These interventions might, or could be, covered by Medicare or private insurance options. This is a great way to keep you in the care continuum with a solution that may provide benefit to you and help you postpone, perhaps even avoid a joint replacement.

On another note concerning the Personalized Stem Cell Trial, we have high hopes our office will restart the study enrollments in mid-May.  My co-investigators will update each of you already enrolled on the timetable and next steps.  They realize that those of you already screened will need eligibility reconfirmation and a clinical pathway re-run.  We will want to make sure you still qualify and it is safe to proceed including COVID-19 testing.

PSC also has other news on the COVID-19 front.  They filed a second Investigational New Drug (IND) application last Wednesday; and, hope to hear from the FDA this week enabling the treatment of COVID-19 patients with allogeneic adipose stem cells while doing their part in the war.  The data from the outcomes will also help with upcoming Personalized Stem Cell studies. 

Last of all, PSC is considering filing next week to use Stromal Vascular Fraction, that is stem cells from fat (SVF), to treat individual COVID-19 patients after they leave the hospital focusing on the lung damage and fibrosis. Scientists are learning that patients requiring respirators during hospitalization may be at risk for permanent lung disease.  An Adipose derived stem cell intervention for active or prevention of lung disease would work nearly identical to our knee study but patients would receive their SVF via intravenous infusion instead of joint injection.
